Adjustment Report (Form C-5)

Purpose of form: This adjustment report shall be used only for the purpose of adjusting a quarterly report previously filed and that has been found to be in error with respect to wages reported.

If this adjustment report indicates an under-payment, attach remittance for the additional amount due. If an over-payment has been made, credit for the over-payment will be issued.

Requirements: If the amounts reported on Form C-4, Wages List for any individual employee is affected by the adjustment to this quarter, attach a Wage List Adjustment Schedule, Form C-7, showing adjustments of "Total wages" (if any) for each individual involved.

A separate report is required for each calendar quarter to be adjusted.

The adjustment report MUST BE signed by an Owner, Officer, Partner, or individual with power of attorney for the business.
