Purpose of form: This form provides employers, accountants, bookkeepers and software companies the specifications for preparing a computer generated Employer's Quarterly Report.
Who can use this form: This form can be used by any individual or organization wanting to use their computer to complete the Employer's Quarterly Report. To report more than 5 employees, use the Employer's Quarterly Report Continuation Sheet (Form C-4) to list all wage data rather than listing part of the employees on the Form C-3. See Computer Specifications for Form C-4 Wages List (Form C-4SP) for details.
Requirements: The printer specifications require that data is to be printed using black ink
at 10 characters per inch, 6 lines per inch, double spaced. The paper specifications require that
if paper is to be other than TWC provided forms, it must be white, 20 lb, 8 1/2 X 11, OCR grade -
void of horizontal or vertical lines. The "shade" of green for the form is PMS354 with 20
percent shading.
Anchor Points:
Each leg of anchor bar is 1/4" in length. Thickness of bar is minimum 0.03" and maximum
0.05".
